Early Intervention
The Early Intervention Program begins with a referral to the Nassau County Department of Health (NCDOH) to start the evaluation process. The referral can be made directly by a parent by calling NCDOH at 516-227-8661. The HLVS can also make the referral for the parent.
The evaluation is conducted by a multidisciplinary evaluation team. At least two evaluations will be conducted, one in the specific area of concern and one that serves as a baseline to see exactly how the child is developing across all domains of development. The parent is given the opportunity to select a New York State approved evaluation site to conduct the evaluation. The HLVS is an approved evaluation site.

Preschool Program (Children Ages 2 years 6 months-5 years)
Each school district's Committee for Preschool Special Education (CPSE) determines a child's eligibility for special education services. If the CPSE determines that a child is eligible for special education services, it is their responsibility to make the referral to an approved program such as The Hagedorn Little Village School (HLVS). HLVS is under contract with Nassau and Suffolk Counties to provide special education services for preschool children.

The CPSE Evaluation Process to determine Eligibility for Special Education
The CPSE Evaluation process determines whether a child 2 years 6 months through 5 years is eligible for special education services. Parents may contact their school district's CPSE to request an evaluation. Evaluations may assess a child's level of functioning in any one or all of the following developmental areas: speech and language, fine and gross motor skills, cognitive, sensory integration, behavior, or hearing. The school district will request a parent's written consent before the evaluation can begin. There is no direct cost to the parent for this evaluation. Preschool special education services are funded by county and state funds.
The CPSE may provide the parent with a list of approved New York State Education Department Preschool Special Education Evaluators. HLVS preschool evaluation site is under contract with Nassau and Suffolk County Department of Health. If a parent chooses HLVS, the referral will be sent to the school and the parent will promptly be contacted to schedule their child's evaluation(s).

CPSE Meeting to Determine Eligibility for Special Education Services
After the evaluation process is completed a copy of the evaluation reports is given to the parents and reviewed with them. A copy is also sent to the CPSE. Parents are then invited to a scheduled CPSE meeting. The evaluation results are reviewed at the meeting and the CPSE will determine the child's eligibility for special education services. If the child is eligible for special education services, the CPSE develops an Individualized Education Program (IEP) to meet each child's unique needs.

CPSE Referral Process to Special Education Programs
The CPSE Chairperson will send a referral packet, which includes a child's IEP and evaluations to approved programs such as HLVS.
The admissions coordinator at HLVS will acknowledge the receipt of the child's referral information and invite the parent to visit the school. The parents will have the opportunity to learn about the programs and services and to tour the facility. The CPSE and the parent will be notified if there may be an available, appropriate placement and a screening appointment will be scheduled.

Elementary School Program Referral Process (Children Over 5)
The school district's Committee for Special Education (CSE) determines eligibility for special education services for children over 5 years of age. If the CSE determines that a child is eligible for an out-of- district special education program, it is CSE's responsibility to make the referral to an approved program such as The Hagedorn Little Village School (HLVS). HLVS is chartered by the New York State Board of Regents and provides services to students residing in Nassau and Suffolk Counties as well as New York City and Westchester.
